You can try to open it using ADOBE ACROBAT READER 6.0. Since I used version 6 to create this file, there might be some incompatibility issues. Thanks.

Hi, how do you input chinese to this form, I tried NJ star, but can't see chinese characters. What do you use to fill in chinese? thanks.

To input Chinese, may I recommend a free and very powerful product called unispm. Its Chinese name is Tshinghua Zi Guang (Purple Light). Do a google on it and you should find plenty of links. You can try one from this link http://down1.tech.sina.com.cn/download/downContent/2004-03-16/4240.shtml
